ACKNOWLEDGEMENT OF RISKS AND CONSENT TO PARTICIPATE

I hereby acknowledge and accept the risks inherit in the requested activities and assume full responsibility for above mentioned child’s personal health, medical, dental and accident insurance.

I give my permission for above mentioned child to participate in Filmberries Studio Film Birthday Workshop, physical activities, off-site activities and intramural activities.

I do consent and allow my child to be filmed, videotaped and/or photographed for use by Filmberries Studio, its partners and the media. I also allow my child’s work product to be featured by Filmberries Studio (this will include our projects, promotional materials on social media and website as well as various Film and Art Festivals). YOUR CHILD WILL BE ACTING IN THE SHORT FILM
